How to get more chiropractic patients.

Work out which of three problems you actually have before you spend anything. Either people near you do not know you exist, or they find you and do not book, or they book once and never come back. Those need different fixes, and spending on the wrong one makes the waste bigger rather than smaller.

This is not a list of ten tactics. It is the order we would fix things in, why that order, and roughly how long each part takes.

Start here

Three problems that look identical from the inside.

A thin schedule feels the same whichever cause it has. The tests below take about an hour and will tell you which one you are actually dealing with.

  1. 01

A reach problem: not enough people know you exist

Test it by searching what a patient would search. "Chiropractor near me", then your city plus the conditions you treat. Do it signed out, on your phone, off your practice wifi. If you are not in the map results and not on the first page, people looking right now are not finding you. That is a reach problem and it is the slowest of the three to fix.

  2. 02

A booking problem: they find you and nothing happens

Call your own practice at 12:30 and again at 4:45 on a Friday. Time how long until a human responds, and note what happens if nobody does. Then book yourself an appointment from your phone as a new patient. If either takes more than about a minute, or ends in voicemail with no follow-up, you have a booking problem. It is the most common of the three and by far the cheapest to fix.

  3. 03

A retention problem: the back door is open

Pull a list of patients who were active six months ago and have not been in since. If that list is long, you do not have a new-patient problem at all. You are filling the front while the back empties, and buying more new patients is the most expensive possible way to fix that.

Most practices have two of these

Usually booking and retention, while believing they have a reach problem. That is why the instinct to advertise more often makes things worse: it pours more people into a path that already leaks. Find the cold spots before you spend on reach.

The principle

Recover, then get found, then create demand.

The order is not a preference. It is a sequence of increasing cost and decreasing certainty. Recovering a patient who already called you is nearly free and nearly certain. Getting found by someone searching right now costs more and takes months. Creating demand in someone who was not thinking about their back at all is the most expensive thing on the list.

Working in that order also means each stage makes the next one worth more. Fixing the booking path before you advertise means every ad dollar afterwards lands somewhere that converts. Doing it the other way round means paying to discover your own gap.

Weeks, not months

First: recover what you are already losing.

Everything here works on people who already chose you once. No awareness needed, which is why it is fast and why it should always be first.

  1. 01

Text back every missed call, automatically

Someone in pain calls three practices and books with whoever responds first. A missed call at lunch is a patient lost by mid-afternoon, and you never see it happen, which is why the loss stays invisible. An automatic text within seconds ("Sorry we missed you, would you like the next opening?") turns a share of those back into bookings. This is the single highest-return fix in the profession and it can be switched on in days.

  2. 02

Reactivate patients who have gone quiet

Take that six-month lapsed list and contact it properly. Not a blast. A short, specific message that acknowledges time has passed and makes rebooking take one tap. Most practices have several months of revenue sitting in that list and have never once asked for it back.

  3. 03

    Rebook no-shows the same day

A no-show is not a lost patient unless you treat it as one. An automatic same-day message with a booking link recovers a meaningful share of them. Doing it by hand means it happens when the front desk is quiet, which is exactly when it is not.

  4. 04

Close the gap in care plans

A patient misses one visit, then two, then stops. Nobody calls, because nothing flags it. A reminder before the appointment and a follow-up after a missed one keeps plans on schedule, and a patient who completes their plan is worth more than a new patient and costs nothing to acquire.

How the automated front desk works

One to six months

Second: get found by people looking right now.

Slower than recovery, cheaper than advertising, and it compounds. Someone with sciatica is already searching. The job is being the answer they find.

  1. 01

Fix your Google Business Profile properly

The highest-leverage hour in local marketing, and most practices have never spent it. Correct hours including holidays, every service listed with the words patients use, real photos of the actual office and providers, the booking link in the right field, and questions answered. The map results decide more chiropractic visits than your website does.

  2. 02

Build a page for each condition you treat

One page per condition, not one page listing all of them. Sciatica, lower back pain, neck pain, headaches, prenatal care, sports injury, whatever you actually treat. Each page answers what it is, how you treat it, what a first visit involves, and what it costs. This is what ranks, and it is also what AI assistants quote when someone asks them for a recommendation.

  3. 03

Ask for reviews consistently, not in bursts

Steady and recent beats a big old pile. Ask at the moment of relief, not by email three days later, and make it one tap. Answer every review including the bad ones, because how you answer a complaint is read by every prospective patient who scrolls past it.

  4. 04

Make the same facts true everywhere

Name, address, phone and hours identical across your site, Google, Apple Maps, Yelp, health directories and insurance listings. Inconsistency quietly suppresses local ranking, and a wrong phone number in a directory you forgot about is a patient calling a dead line.

  5. 05

Make the site answerable, not only readable

Plain answers near the top of each page, clear structure, marked-up practice details, no critical information trapped in images. Patients increasingly ask an assistant rather than scrolling results, and assistants quote pages they can parse. Nobody can promise you a place in an AI answer, but this is the work that makes you quotable.

Third: create demand that is not there yet.

The most expensive tier, and the one most practices start with. It works, but only once the path behind it holds.

  1. 01

Paid search, for intent you can buy today

Someone typing "chiropractor near me open now" is ready. That intent is expensive per click and converts fast. Send it to a page about the specific thing they searched, never the homepage, and track calls as well as forms or you will misjudge the whole channel.

  2. 02

Paid social, for people not looking yet

Cheaper attention, colder audience. Better for a new practice building awareness than for filling next week. Expect to pay less per click and more per patient, and judge it on booked visits rather than engagement.

  3. 03

Short video, because it is what gets shown

A 30-second answer to a question patients actually ask, filmed on a phone in your own office. It travels further than a written post and it makes you a person rather than a listing. The constraint is not production quality, it is doing it every week.

  4. 04

Referral relationships, which are slow and durable

Physical therapists, massage therapists, trainers, running clubs, the local sports teams. Slow to build and almost impossible for a competitor to take from you, which is the opposite of every paid channel. Nobody automates this. That is why it keeps working.

Every patient here still lands on the front desk

Which is why this section is third. Advertising into an unanswered phone is buying people the practice will lose at the last step, and the more you spend the more it costs you.

Set expectations

How long each of these actually takes.

These are the timelines we set with clients in writing during onboarding. They are our own expectations, not industry data, and they move against your starting point.

Straight talk

Four things we would not bother with.

All four are common in chiropractic marketing. None are frauds. All four are worse than what you could do with the same money and time.

  • 01

    Repeat new-patient specials

They fill a month, then fade, and every round teaches your market to wait for the next one. They also select for price-sensitive patients who are least likely to finish a care plan. Fine as an occasional event. Ruinous as a strategy, because eventually your normal price looks like the exception.

  • 02

Screenings at every event in town

A whole Saturday for a handful of leads who mostly do not convert, while the reactivation list you never sent sits in your system. If you enjoy them and they build local relationships, keep going. Just do not count them as your patient acquisition plan.

  • 03

Posting daily on social to stay visible

Volume is not the lever. One genuinely useful video a week beats seven stock graphics, and the seven graphics cost more time. Social works for chiropractic when it answers real questions in your own voice, which is slow to produce and impossible to fake at daily cadence.

  • 04

Chasing a keyword ranking as the goal

Ranking first for a phrase nobody in your city types is easy and worth nothing. Ranking is a means to being found, and being found is a means to being booked. If your reporting stops at the first one, you cannot tell whether any of it worked.

Straight answers

Questions chiropractors ask.

How do I get more chiropractic patients?

Work out which of three problems you actually have before spending anything. Either people near you do not know you exist, or they find you and do not book, or they book once and never come back. Reach, booking and retention need different fixes, and spending on reach when the real problem is booking makes the waste bigger rather than smaller. Fix the booking path first, because it is the fastest and it multiplies everything you spend afterwards.

What is the fastest way to get more chiropractic patients?

Answer the calls you already miss, and contact the patients who have gone quiet. Both recover people who already chose you once, so neither needs new awareness. Automatic missed-call text back can be working within days, and a reactivation campaign to lapsed patients usually produces bookings inside two weeks. Everything that creates new demand is slower and more expensive than recovering demand you already had.

How long does chiropractic marketing take to work?

It depends entirely on which lever you pull. Missed-call recovery and reactivation show results in days to weeks. Google Business Profile work and review generation take about one to three months. Local SEO, treatment pages and AI search visibility take three to twelve months. Paid ads produce clicks immediately but usually need four to eight weeks to become efficient. Any agency giving one timeline for all of it is guessing. The full breakdown is in the timeline table above.

Do new-patient specials and screenings work for chiropractors?

They fill a month and then fade, and each round teaches your market to wait for the next deal. They also select for price-sensitive patients who are less likely to complete a care plan. They are not useless, but they are a way to create a spike, not a way to build a practice, and running them repeatedly makes your regular pricing look like the exception.

Should chiropractors run Google Ads or Facebook ads?

Google Ads reaches people already searching for relief, so intent is high and it converts faster. Facebook and Instagram reach people who are not looking yet, so it costs less per click and more per patient, and it works better for awareness in a new market. If your booking path is not fixed, neither will work well, because you will be paying to send people into the same gap they were already falling through.

How many reviews does a chiropractor need?

There is no threshold number worth quoting, and anyone giving you one made it up. What matters more is that reviews are recent, steady and answered. A practice with 40 reviews spread across the last year reads as busy and current. A practice with 200 reviews that stopped two years ago reads as a practice that used to be busy. Ask consistently rather than in bursts.

Can I do all of this myself?

The first tier, yes, and you should start today whether you hire anyone or not. Missed-call text back, the lapsed patient list and your Google Business Profile are within reach of any practice with an afternoon and some software. The second and third tiers are where it becomes a job: condition pages, listings, reviews and ads need doing every week, and the week you get busy is the week they stop. That is the honest case for hiring someone, and it is a capacity argument rather than a secret-knowledge one.
